OpenEMR API
Integuru generates a production-ready HTTP API for OpenEMR, the open-source EHR, for teams who have a login on a hosted instance but no administrator to switch on its REST or FHIR API. 1 verified action lists patients with limit and offset pagination, verified against the public demo, with chart and scheduling actions available on request against your own instance.

Don't see the action you need?
The actions above cover the most common OpenEMR integration patterns, but they're not the full picture. Integuru can build additional workflows for any capability the platform exposes. Our Managed Service handles scoping, building, and maintaining custom actions on your behalf, so just let us know what you need.

Does OpenEMR have a public API?
Yes, with a condition. OpenEMR ships a standard REST API and a FHIR R4 API, but both are disabled until an administrator turns them on under Connectors, configures the site address for TLS, and registers your application at the OAuth2 registration endpoint. Integuru generates an HTTP API from a plain OpenEMR login for the case where that administrator is not you, with 1 verified action.
| OpenEMR REST and FHIR APIs | Integuru for OpenEMR | |
|---|---|---|
| Access | Administrator enables the API, sets the site address, registers your OAuth2 client | Your own OpenEMR login, no admin change |
| Covers | Patients, encounters, observations, medications, CCD generation and more | list_patients today; further actions on request |
| Best for | Teams that run the OpenEMR server | Vendors with a login on a hosted instance they do not administer |
If you control the server, the official API is the right tool and it covers far more than this page. The gap Integuru fills is the community clinic on a hosted OpenEMR whose vendor will not enable the API, or the pilot that needs to read a roster this week. Calls are direct HTTP requests, authenticated, using your own credentials, with the login handled for you.
What you can do with the OpenEMR API
list_patients returns the patient roster with limit (default 10, maximum 100) and offset (default 0). It was verified against the public demo instance, so a developer can exercise it in the CLI before any credentials from a customer exist. In a multi-EHR pipeline it plays the same role as the roster reads on the DrChrono and Practice Fusion pages: the lookup that maps a patient in your system to a chart in theirs.
Who uses this
Health-tech vendors piloting against OpenEMR before a customer's IT team has approved an API client use the demo to prove the workflow, then connect the customer's login. Community clinics on hosted OpenEMR without administrator rights get a roster export for a care-gap or outreach tool. AI agents that need to confirm a patient exists before drafting anything call list_patients with a small limit and page until they find the match. Teams building on a headless EHR alongside OpenEMR pair this page with the Canvas Medical integration; the healthcare industry page covers the rest of the stack.
Setup notes
Integuru authenticates against an OpenEMR instance with your user; the verified base URL is demo.openemr.io/openemr, and your own instance is connected the same way. Keep limit at or below 100. Because the action reads the same endpoints the OpenEMR web app uses, a self-hosted instance on an older release may differ; the on-call team handles that on Production plans.
Get started
Install the CLI and connect an OpenEMR login:
npm install -g integuru
Or sign in at app.integuru.com and add OpenEMR. To move past the roster into encounters, appointments or chart writes on your instance, the Managed Service scopes and builds those actions.

Good to know
Before you start
- Can I use it today?
- Yes. Every action listed above has been verified against live OpenEMR traffic. Install the CLI or sign in at app.integuru.com, connect your OpenEMR account, and start calling the endpoints.
- What if OpenEMR changes or breaks?
- On Production plans the Integuru team is on call around the clock, with auth auto-healing included, so target-platform changes get fixed by Integuru rather than by your engineers.
- What does it cost?
- Start on the Free plan at $0/month with 100 API calls/month. Production starts at $499/month with 24/7 on-call maintenance and auth auto-healing.
